| LBHS ATHLETIC PHYSICALS Tuesday May 24th and May 26th ONLY $25.00 Below are all necessary forms, please print out, sign and return (9 total PDF files) The 2011/2012 LBHS Athletic Physicals will be conducted in the LBHS
Quad, next week, immediately after school:
Tuesday, May 24th and Thursday, May 26th
This year, boys and girls can attend on both days. Students must bring all completed forms and a check for $25, made
payable to “LBHS Athletic Boosters,” or cash, on either Tuesday, May 24 or Thursday, May 26 after school in the quad.
Students will be processed on a first come, first served basis.
This year, we urge families to take advantage of the Cardiac Screening
that is being offered. You MUST sign up ahead of time for the Cardiac
Screening. Go to www.heartfeltcardiacprojects.org to make your
appointment. You can also find more detailed information about the
screening process on the attached Heartfelt flyer. Heartfelt Cardiac
Screening requires an $85 tax-deductible donation.
